In the Itivuttaka, sections 20 and 21, the Buddha asserts that one's state of consciousness at the moment of death is what links one to the next life. And there is no soul or self to transmigrated between lives, so I imagine it as a sort of generic subjective continuity where the next life in a stream of consciousness is one that happens to be "vibrating on the same frequency" as the former.
